Once you've received your lab order from your LifeMD affiliated provider (your lab order will be available within your LifeMD patient portal and sent via email) and you would like to schedule an appointment at a Quest Diagnostics location, please follow these steps:
1. Go to https://appointment.questdiagnostics.com/as-home, and click the “Schedule appointment” button.
2. This will pop up, where you can either sign in to an existing account, create a new account, or continue as a guest.
3. Whether you sign in to an existing account or continue as a guest, the next screen will look like this. Go ahead and enter all information; then click the green “Continue” button.
4. You’ll then be brought to this screen. Select “All Other Tests”, and click the Continue button.
5. You’ll then be brought to this screen, where you can use the scheduling tool to book an appointment for the date, time, and lab location of your choice.
6. There is also the opportunity to opt for the Quest Mobile™ option (at-home sample collection by a phlebotomist). This comes with a $79 in-home sample collection fee. If you select that option by clicking the “Book now” button, an "Advanced billing notice" message pops up. (Note: This Quest Mobile service is not covered by LifeMD. If you choose the Quest Mobile™ option, you will be responsible for the cost.)
7. If you click the Continue button, you are brought to a site outside Quest Diagnostics to set up your Quest Mobile at-home appointment and submit the required upfront payment.
8. If you did not select the Quest Mobile™ option when scheduling and are opting to go to a Quest Diagnostics location, please do the following: Select the date, time, and Quest Diagnostics location that works for you. Then, click the Continue button.
9. Next, you will be asked about primary insurance information.
If you select “I’ll save time and provide it now”, you’ll be brought to this screen to enter your insurance information:
If you select, “The person being tested will bring it to the appointment” or “The person being tested doesn’t have insurance”, you’ll be prompted to click the “Review Details” button where you can review and confirm the details of your upcoming appointment before clicking the “Confirm” button to confirm your appointment.
10. Click the green "Confirm" button to confirm your appointment at Quest Diagnostics.
